Notification Form (PDF) | Customers must complete the Notification form to request Gas Transport Service or convert back to Company-Supplied Gas Service. Ameren must receive the form directly from the customer prior to July 1 for a change in service to be effective Nov 1, or prior to June 1 for schools requesting aggregation transport service to be effective July 1. |
Supplier Authorization Form | Customers must complete the Supplier Authorization form to allow their marketers the authority to nominate on their behalf and to access their account information on the Gas Transport System. |
Gas Transport Nomination Form | Customers or marketers use the Gas Transport Nomination form when they do not have access to the Gas Transport System and need to get the nomination submitted in a timely manner. |
Account Agent Designation Statement (also see Designation of Account Agent Policy) | Customers must complete the Account Agent Designation Statement to establish an Agency relationship with a third party to allow them to make decisions related to the customer’s account(s). |
Termination of Account Agent Designation (also see Designation of Account Agent Policy) | Customers use the Termination of Account Agent Designation form to end an existing Agency relationship with a third party. |
Release of Utility Historical Information | Customers use the Release of Utility Historical Information form to give Ameren permission to release historical information to a marketer. |
